Neurosequential Model of Therapeutics© Assessment Package

  1.  Screening (Phone/Skype:  30 minutes)

  • Is this client appropriate for the NMT process?

  • What are the current/past interventions?

  • Quick developmental history

  • If interested in proceeding family receives NMT literature

  1.  Initial Assessment (in person: 1.5-2.5 hours)

  • NMT Client Workbook

  • Trauma-Informed Occupational Therapy Assessment

  • Interview with family and desired team members

  1. Results Appointment (in person or via zoom/phone: 1-1.5 hours)

  • Review and receive colored copies of the functional brain map

  • Receive a detailed report with individualized recommendations

  1. Follow up Appointment 3-6 weeks after results session (30-60 minutes) and follow up sessions ever 6 months as desired by family

  • Office, phone, or Zoom
